It can be anything that actually takes up an item slot, equipment, or goes into your collectible tab like pets, toys, mounts, heirlooms, and item appearance (full appearance set does count as one item). Faction restrictions do not apply, you want a Horde Bike as Alliance its yours, you want the Bee mount as Horde before its released its yours!
The Anzu mount. I’ve been farming it for years and it has never dropped.

Its hard to choose. Either black proto drake, Zin’rokh, or the MoP challenge weapons.
Edit: upon further thought i decided on the black proto drake.

I had that set, and got rid of it before transmog was a thing, so i would love to have it back.
I’d also just like to be able to use my AQ mounts in the world, and not just in a place I don’t go anymore.

I would take the Baby Blizzard bear.
My very first character had that pet (which was my very first pet). I had made the character on my sister’s account when I was visiting her. Neither of us knew at the time that such a thing was frowned upon, and I didn’t have internet available where I lived so I couldn’t have had an account of my own anyway.
When I was finally able to get a WoW account of my own (using satellite internet, loll!), I thought I could transfer my character from her account. Alas, it was not to be. So, I lost my character and the bear.
I really miss that little guy.

It’s a toss up between the original Frostmourne and maybe Atiesh. I’m not actually super fond of the way that staff looks though…maybe instead I’ll take a Dragonwrath (legendary staff) that scales with my average item level forever so I can always use it and it’s dragon form. I mean I do already have the regular one but something that has such a cool on use ability should never be outleveled!
Have that on my first ever toon. My nelf hunter. Wish it had the origional skin though.
