Ok, So I've already submitted a bug report on this issue but I'd like to get a feel of how many cases of this are out there.
Basic story of the bug is that you have a multi follower mission.
Add first counter, chance success = 70%
Add second counter, chance success = 20% or less
Can someone please explain to me how adding a counter can DECREASE the chance of success of a mission.

If it's a stealth mission (which you can see if you look at the top left corner of the mission display), any non-champion unit you add to the mission will decrease its success chance. Yes, this is incredibly stupid.

Probably is a "stealth" mission. As in if you added any follower as a counter, it decreased the success chance by 100%.

Have you checked to see if that mission is a 'Stealth' mission? Missions can have an extra modifier to them, such as Elite (which adds 25% uncounterable difficulty) or Stealth (which adds massive difficulty increase if you use any minions instead of followers)

Does this happen on all missions.. or just a select few?
Perhaps I may shed some light on this issue. You need to look at the mission TYPE. They give certain bonuses .. Such as Long campaign, will give you a higher % per vitality heart of troops you send.
Now your issue must be with the "Stealth" mission type. If you add troops to the mission, you will DECREASE your % by 100%. So if you countered something to boost it by X amount, you will still net a loss overall.
Double check the mission type if you haven't! It confuzzled me a few times before I noticed what was going on myself. Otherwise.. I have never seen such a bug with the mission tables.

Sounds like you hit a stealth mission. They are a royal PITA.
