I think this is technically a quest, so I decided to ask here.
I am working on the All Consuming achievement with 6/7 completed. However, as of the last week or so it seems like Consuming Maws are not spawning for me.
I’ve tried a Horde character, Alliance character, characters that have completed lots of BFA content, characters that have just unlocked assaults, while being on the “Assault: Black Empire” quest, while not being on the “Assault: Black Empire” quest, warmode, no warmode, just after server reset, during off peak playing hours, and camping out for hours…
These suckers are not spawning for me.
I’ve checked wowhead, reddit, and here on the forums to see if people are having the same issue, and there are a couple of posts from folks confirming that they got them to spawn, but something is definitely off for some people, myself included. Perhaps a bug?
If you HAVE seen them, and can recall the conditions that you saw them under (see my above testing for examples), can you please comment? Also if you can note the number of cleansed maws you have for the “All Consuming” achievement, that would be helpful too. I want to see if there’s a common thread or something I’m missing. 
Haven’t seen any myself since last week in the Uldum assault. I’ve got 2 left to go and can’t seem to get them to spawn at all either. Seems bugged for sure 
I completed the achievement on Dec. 7th on an Alliance character that had done a good bit of BFA content, during the corrupted assaults, sometimes in war mode, sometimes not, sometimes off peak hours, sometimes not, mixed between Uldum and the Vale. I never camped them, I just flew by the spawn locations whenever I was doing quests. Sometimes I would circle a few times. I had a handful of them done from BFA already.
They definitely don’t seem to have the short respawn that was suggested on some wowhead comments. The funny part is that when I was grinding rep after I finished the achievement, I found so many of them by accident lol. It seems to appear like a world quest objective, so I’m not sure if there is a limit per day? I read that not all locations are marked as active every day.
I tried to check the locations on an alt to get more, but that seemed like a waste of time, so I just found them naturally while doing other assault achievements on my main assault character. I almost thought it was bugged at times based on my difficulty finding them some weeks, but then they would appear. I do not know if there is a rhyme or reason to their spawn. It’s possible that is the case now, but I took a break once I finished my assault achieves last week, so haven’t checked the zones.
If you have other things to do in the assault, I would definitely focus on that and you’ll hopefully encounter them.
I started working on the meta achievement for BFA one week before patch 11.0.7. They were spawning for me previously… I started with 4/7 was able to get up to 6 last week, but it seems like they just stopped spawning around 12/21.
I am working on other parts of the achievement, but this quest event is quickly becoming one of the last things I need.
I know there were definitely some weeks before the patch that I never saw them. Looks like I finished my reps on 12/15 and 12/18, but I saw several in the last push, which was abnormal. I’m not certain how many were before or after the patch, though. Hopefully you’ll find the ones you need! If I happen to see any I’ll report back.

In the same boat. Havent seen them spawning for at least 4 days. (last 2 days of uldum and first few days of Vale. At 6/7 for the achiev.
