I have a pretty good list of corrupted gears and I’d hate to cleanse some of them but in sake of optimization… I will have to.
What’s the best corruption combination?
I’d like to keep it under 40

The ideal current set-up for yourself is whatever sims highest. There is an option in raidbots to sim everything both corrupted and cleansed so it’ll tell you exactly what to go with. This could cause you to make some poor long term cleansing decisions though.
What spec are we talking about? What corruptions do you currently have access to?
Raid? Single target or multi-target fights? M+? The ideal set up is going to include a variety of corruptions you switch between depending on the fight/situation.
Expect GW… we all want it, but it doesn’t want us
For AoE Twilight Devastation 3 is unbeatable. Void Ritual 3 is also great.
People often scoff at Void Ritual but the stat boost it gives is huge and it’s pretty reliable compared to random procs of Twisted Appendage or Twilight Dev.
Gushing Wound is strong for both ST/AoE but it’s quite rare in my experience.
The fact that there’s a guaranteed Twilight Devastation corruption in N’yalotha makes TD easier to acquire. Finding a good item with Gushing Wound is much harder since it’s a random corruption chance.
SIngle target for me: Rank 3 infinite stars + gushing wounds
AOE: Rank 3 Twilight Devastationi + gushing wounds
This is technically correct but somewhat misleading. The trouble with Infinite Stars is that a true Patchwerk single target fight is a rare thing. The only one in Ny’alotha is Shad’har. Every other fight has adds and target swaps, and every time Stars procs on a new target it’s doing the low base damage and not building the debuff stack on your main target. Those put together really lower its effectiveness.
Infinite Stars is great on Shad’har. It’s strong on Maut, though you risk blowing yourself up on the damage reflect. For all the rest? It performs well below the sims values. I’ve been getting better results from Twisted Appendage 3, where it just does flat damage and can swap targets if it procs on an add that dies quickly. Now maybe if you’ve got Stars 3 on a 470 or 475 it’ll still pull ahead, but if it’s on a 460 or you’ve only got Stars 2? Be open to alternatives.
ETA: Really, trying to match the simplified sim fight types to the complex actual play raid bosses is hard. I just did some rechecking of my gear sets against multiple fight types. On pure single target, the Twisted Appendage set sims ahead. On Hectic Add Cleave the Twilight Devastation piece sims ahead, but I’m cautious of depending on the RNG to give you procs while the adds are up. On a single Cleave Add sim the set that ranks highest is my “All the small but high value” Corruptions set, with GWx2 and Strikethrough 3 and more.
tl;dr There’s no all purpose solution, and you’ll get the best results from having multiple sets you swap between depending on the fight.
I have at least 25+ corrupted pieces in bank not including what is in my bags and what i have equipped. I have yet to find 1 piece of this >:(
