Has been happening since 10.2.5, if not earlier. Happens across any and all of my toons - I pick up a quest, but it doesn’t automatically track. Have to open my quest log, find it, and manually track it. Turn it in, pick up next quest, same thing.
Do you have a list of quests on the right side of your screen? Does it set tracking to a different quest? That is what it does for me and it is annoying.
I set tracking on a quest, complete that quest and pick up a new quest and head toward the quest arrow only to find out it isn’t the quest I barely picked up but one that is on the other side of the continent.
I do have my quest tracker on the right side of the screen. It maintains achievements I have tracking, but no quests tracked until I manually track them. New quests don’t automatically track like they should.
