Apholeias, Herald of Loss, the special encounter in the maw near coords 21, 40, is not functioning as intended and is not spawning under any circumstances. Have tried doing so with multiple groups of players, both Warmode on & off, during different covenant assaults as well. It still seems to be bugged despite all of this.

Same issue for us. We have tried all three, are listed as up but still not spawning. Really frustrating with how many things have been buggy lately.
oddly enough ive killed this guy 3 times but havnt been able to get the herald of pain to spawn.
Had the issue as well and also at another spot that needed 4 to summon the boss, even the gold star was up.
One comment on wowhead was to join a charger group and sure enough that worked to phase us off the buggy server we were on and onto a server with a working system.
Same issue here. Doesn’t appear that this one, nor Ekphoras, Herald of Grief, is spawning correctly, given the proper summoning method of 4 people standing on each of the lighted pads to summon.
