Inquisitor Jociphine and all other non-guard NPCs not loading for one of my characters. The character in question progressed to “Fall of the Grand Crusader” before completing New Avalon. My guess is that is the issue; I killed 5 bosses bosses Scarlet Enclave with this character before ever equipping the disguise and activating the Scarlet Insignia item. That’s my only hypothesis but in any case, my 3rd character can’t turn in the completed “New Avalon” and no other quest giver NPCs exist at the base camp right next to the raid for that character.
I’ve tried reloading, I’ve tried re-starting the game. Tried turning on the disguise and off in Tyr’s hand and New Avalon and going back tot he base camp. Can’t get the NPCs to show. Even other players around me when they target the NPCs, I can’t see them in my target-of-target UI. Booting with all addons OFF didn’t fix it either.

I have this same exact issue. If you progress the Raid Questline and kill Balnazzar it seems to cause the NPCs to disappear. We killed the first 3 bosses and I hadn’t progressed “New Avalon” yet. I had Red=Dead completed, but didn’t turn it in. I removed it from my Quest log hoping it would fix the visual bug. Nope.
I had the “New Avalon” quest in my log, but deleted the Insignia for more bag space. I went to speak to Jociphine to get a new one and they weren’t there.
Technically it makes sense, because the NPCs you need to speak to are either dead in the raid or standing somewhere inside, but it breaks the outside quest chain, so it’s a pretty ridiculous bug. Not sure if the NPCs will be back after reset because Beatrix is no longer dead that week. I had to use Item Restore to get the Insignia back. If not, I’d be screwed over and not able to Disguise myself.
Same problem, seems consistently replicated.
Step 1: Don’t complete “New Avalon” quest from Josephine.
Step 2: Complete “Decapitating Strike” raid quest to kill Balnazzar
Step 3: NPC’s at forward camp are completely gone.
Step 4: Unable to obtain, progress, or complete any quest from Inquisitor Josephine.
Step 5: Clearing cache, WTF/WDB, Interface, and fresh-installs do not work.
No work around available. Issue is Blizzard side with phasing for quest progression. NPC is not found anywhere else in game currently.
Edit: This is fixed, but there is no catch-up in place.
