I had the same issue, after trying all the usual tricks, logging out, reloading, abandoning etc. nothing worked
I read in one of the comments that a pre-req is Sojourner of Thaldraszus was required… which was wrong (I spent half a day getting through all the Sojourners)
TLDR: What did finally work for me, is I followed the campaigns (I reached 70 without going through the storyline) - so I did all the campaigns to get to unlock Adventure Mode (Achievement is: “Just don’t ask me how to spell it”
This finally worked for me, I went back and the I was able to learn the fate of whatshisname… and it was kinda fun but painful. Good luck!

I can attest that this “solution” didn’t work for me. I’ve finished the campaigns on two of my toons, and have the ‘Just don’t ask me how to spell it’ achievement complete and adventure mode unlocked. Even with this accomplished, the quest is bugged with Wrathion not being at the marked location to finish the ‘Experience the retelling of Sarkareth’s fate’ requirement of ‘A flame, extinguished’.

Not sure if this is going to help anyone, but if you kill Sakarath in the LFR make sure that you go into the Raid and bring both Wrathion and Saballion to Kavia within the ‘raid’ you don’t have to fight anything just bring them there. Then Wrathion gives you the option to retail the story etc… that is my fix that seems to have done the thing correctly.

I also had this issue. I completed “A Flame, Extinguished”, but it still was saying I needed to do it in the achievements. It made it so that I could not progress to the “Symbol of Hope” quest. What I did was complete the “To Tyrhold” quest (seems like Alexstrasza is stuck here until completed). Once I finished that, I moved on to “Symbol of Hope”. After completing that one, it marked “A Flame, Extinguished” as completed. Hope this helps!
