I just played a game where someone was super toxic, so naturally I report, avoid, gg go next. Next game I have them on my team… Is this a bug or what? Because I just ate a -50SR and 10 min suspension rather than play with that guy again.

That would only happen if you avoid after you started a queue for another game, in which case you got into a game before the system had the chance to avoid him.
If you avoided him, then started a queue for a new game, and still got him on your team, that’d be a glitch.
What if I queued, avoided, waited 2 minutes, and then got in a game with him?
Maybe there was a lag somewhere which caused the 2 minute wait, but in reality there was already a game found.
Always avoid first, then queue. That’s what I do.
