There are so many players who abuse, overuse, and misuse, the reporting system, specifically Abusive Chat and Poor Teamwork. What is Blizzard doing to keep these players honest?

Blizzard have said several times that if they find someone mass false reporting they take action on their account.
Apparently not severe enough action, it’s still running rampant
They say that. But can you name one instance, or ever heard of it happening?
