When she shoots an ally, it heals them. When she shoots an enemy, it damages them. My question is how this works considering she is using the same projectile on both?
I think she switches the bullets in lore so letās assume our granny has the fastest fingers in the west and manages to change every bullet depending on the shot.
Donāt worry about that, it just works. If you start with this you will never finish.
How does Widowās rifle work? How does healing or DMG Boost work? How can Sigma affect gravity? Why do have hitscans Falloff but Projectiles not? How can Tracer time travel? Why canāt Junkrat be injured by his mines and grenades?
There is a hell of a lot that, when you think about it, doesnāt make sense.
Iām more amazed by Mercy being able to piece together someone who was obliterated by a Rocket Barrage, or Genji being able to turn Snowball against Mei by basically throwing them back at her (really Snowball, that can hardly be called loyalty).
Maybe she gives her teammates some kind of protection before the battle.

How do barriers work?
Blocks boolets from enemy, but friendly boolets go through the barrier and the tank him/herself.
Maybe she switches the type of shot like one would change a riffle from full auto to semi and she really good at it because reasons?
My headcanon is that her bullets are filled with poison by default, but she injects her teammates with some nanotech stuff that converts it into healing stuff. Her enemies donāt have that, so they get damaged.
But yeah in the lore she has different bullets for healing and damage.
In lore Anas biotic rifle was made with mercys nanobiotic technology and co designed with Torbjorn (also in lore, both mercy and Torbjorn designed the rifle only to heal, and mercy didnāt want her biotic technology being used for harm. She has a comment somewhere between her and ana in game expressing her disapproval for how ana is using it if Iām remembering right)
Assumption here, but, being technological nano biology- i have little difficulty imagining ana āin loreā has a means of allowing the nanites to discern a āfriendlyā target. Maybe itās something already on their system. Maybe itās some kind of radio ID tag you wear the projectile detects when it hits a target, and if it hits a target it where it doesnāt get this signal the nanites are programmed to destroy cells instead of regrow them. Thereās a lot of ways to get around it āin loreā
But to my knowledge they havenāt explicitly stated.
Maybe Someone more familiar with all the lore sources has cross a more finite answer?

I could be some form of acupuncture? since the bullets almost look like syringes. When aiming at an ally, it will always go to the pressure point and heal them, but with an enemy is will always miss the pressure point, putting them in pain/taking damage

i dont think a lot of abilities make any sense so its best just to not worry about it. lol like d.va how come shes immune to her nuke bomb, is her jumpsuit nuke proof? how many ninja stars does genji have in his arm? do they appear out of thin air

I have this theory Anaās allies are injected in their blood with a substance that when reacts with the chemicals in her ābulletā, generate a healing substance to their organism. For everyone else, including innocent civilians and lifeforms, Anaās bullet is toxic!
It could be like a serum given to friendlies before? So like the syringes naturally hurt, but friendlies are given something that reacts with it to make it heal.
