I remember waaaay back in like season 5 or 6 I got a warning for multiple reports. I was one-tricking Bastion and I guess many people didn’t enjoy that

on the forums? i was silenced, and it was only once for by bassing mature language filter (on a thread that got taken down mind you)
Not once in game or forums, even with my low-brow choice of comedy and words.
I got a minor comp ban back in like S14 when my CPU started acting up and I got many reboots in the middle of games.
I got a forum ban for using the Australian term for a male organ without thinking about it.

Never ingame and i was playing a lot of bastion back then too.
In game never, forums twice for very silly reasons.
I was banned twice on the forums for giving controversial opinions that many people don’t like to hear. I felt it was more like censorship than anything.
I was suspended once for three days in the game because I told my (feeding) teammate Reinhardt I had a fantasy of burning him alive in VC. I didn’t say it in a serious way, I was just being theatrical, but the entire team reported me anyway

When the people misconstrue what SHOULD happen as a threat. Smh. I am not Scorpion, I can’t burn them in hellfire myself, trust the process.
I made a little correction. I should’ve been more specific 
I got a warning once back in 2018. Instead of keeping my complaints to myself in Quick Play, I’d air them out in Match Chat for all to see. Things like “[insert expletive] team”, “these guys suck”, “trash DPS” or whatever…I’m not proud of it, but that warning scared me into good behaviour. 
I once got banned for 8 hours after dcing, I dced a couple times in the past month or two but that was it. Such bs lol
