So I signed up for the Twitch Prime promotion for the 3 Wrecking Ball loot boxes and one golden loot box in September and October. The free loot box I received for September however was a regular loot box and not a golden loot box. I thought it might have just been a cosmetic glitch, but when I opened the box it was still only common items. What gives?

Be sure you did not receive a regular loot box through a different promotion (such as an Endorsement Drop). The golden Loot Boxes for Twitch Prime are not available yet as they show to be locked at this time:
your supposed to be able to get one in September but i never received one. here it is September 12th and no golden loot box. really sucks cause there are things i really want that i don’t have the currency for.
can someone tell me if this happens often for things like this?
