I never changed my battle tag on my account after i made it and it said i can change it anytime and first change is free.
Im looking now and it wants to charge for a change
Each Battle.net account is allowed one free BattleTag change. To change your BattleTag, log in to your Battle.net Account Details and click Update in the BattleTag section. If you don’t see the option to update your BattleTag, it means that you already changed your BattleTag in the past. If you would like to change your BattleTag again, you can buy a BattleTag Name Change on the Battle.net Shop.

I edited. There was a period of free change for a bit on top of the supposedly regular one

Depends IIRC if your account get multiple reports by “innapropriate name” some people said they force change your’s BTag.
Then i don’t know. Accounts are free to make now, so i guess that’s a way to swap yout bt for free?
Mine still has the option.

Holy hell, I thought it was $15 for years. It’s $7.25!!! Wtf that’s not even close to liveable
It counts when you made your Battlnet and chose your first name. I am not sure if that applies exclusively to console players, if you are one, but it is a scam and they made me pay for my only name change. It was worth it, however, because I used to have an Overwatch related name and once I got all emo about Overwatch I had to distance myself from it.
