They have to be reported more than 1 time before action is taken. The report system isn’t an automatic thing.

Turn off coms, turn off team chat, turn off match chat.
No one listens to callouts anyway, no one swaps, no one strategizes.
Chat and coms are toxic environments. Just turn them all off, you will enjoy the game a lot more I promise you

Is this a regional thing or something? Because I think I’ve been told this maybe 6 times in the entirety of the time I’ve played OW over thousands of hours, so this seems extremely abnormal.
I play on console, it happens like once a session on console

It could be regional differences and also the heroes you play. Some heroes get more hate than others. I’d say every few months someone tells me some variation of killing myself on this game. But, seeing someone say it to someone else in chat happens pretty often.
Homie I got called the hard r and then got banned for responding back and then my post in the forums got removed for calling out the system.
Blizzard report system is garbage. Not surprising considering their history as a company. They’d rather you be silent

I play on console and the worst thing I remember hearing is a teammate saying he needs to start banning Lucio for these hard stuck Lucio one trick teammates 
that’s way worse than a kys for me
Going to need some proof of that otherwise you can say pretty much anything you want
