So, from reapers interactions we know that he is in pain, my question, can’t he just take paracetamol, Morphine, Codeine, Ibuprofen, Fentanyl or any other pain killers?
Thinking about reaper’s condition, which I think is his cells dying and regenerating quickly, so I assume his body will be able to get rid of the drugs fast, because his liver is working really hard, or even because his digestive system can’t even absorb the medicine.
but this brings me to the second question, what can reaper do?
Does he eat, drink, breathe, go to the bathroom or even sleep?

hypothetically he could have a molecular deformation. from the short story Code of Violence we can guess that moira’s experiments have brought him to the point of having difficulty maintaining his solid state stable, passing in a way that is perhaps a little too directly to the gaseous one.
we can hypothesize that pain is a form of “continuous healing” of the tissues that return to reunite in a solid state, a bit like when you continually reopen the scab from a cut instead of giving it time to reform completely we can think that reaper has tremendous difficulty restoring the wounds until he has a skin completely exposed to a state of “recovery” that however cannot be completed adequately because he cannot maintain the solid state of his body stable.
we could also hypothesize an opposite way: becoming gaseous is the only way to feel relief because the body cannot re-establish itself in its solid state. like a sort of temporary breath.
EDIT: He may also have vocal difficulties, so the mask should allow him to modulate his voice in an intelligible way and with not too much reverberation (again due to a condition of transition to the gaseous state).

I believe he can talk without the mask in a same way that he talks with a mask. As I remember he has talked when Ana removed his mask in that Comic when he ambushed Soldier 76
