There seems to be multiple issues with Lokholars Frigid Winds equipment. It was both triggering when no ability got queued and did not trigger after a minion was summoned (by Jaraxus’s ability).
Reported to me were also cases where “some things which shouldn’t trigger frigid winds, do, such as the bwonsamdi’s deals triggering it twice (thrice with deal of fear), the deathrattle of neeru imps, the end of turn effect of neeru’s imps, an additional trigger for each merc affected by Nefarian’s shadowflame etc.”.
Additionally, summoning minions (N’Zoths spawn) does not activate Medivh’s counter ward.
