I just started playing again after a long break, but i noticed that Fishy Flyer doesn’t always reward its Deathrattle: “Add a 4/3 Ghost with rush to your hand”
I have a clear screenshot of the bug. Happened for me earlier today in arena.

You are exactly right. Deathwarden was the only minion in play when it happened. Don’t know how the screenshot upload works.
