While Running a T21 Nightmare Dungeon, key boss spawned behind the locked door, locking progression

While in Maugen’s Works in a T21 Nightmare, we killed all other enemies, and freed the prisoners, prompting “Slay the Warmaster and collect the Warmaster’s Key”. One problem though, he spawned behind his locked door. Thus preventing progression through the dungeon.

Same thing happened for me while just running Maugen’s Works as a side quest.

Same here, same dungeon, higher nightmare level

Just occured for me too in Charnel House. Clicked the final prisoner and Vardok spawned on the opposite side of his door.
