I did several dungeons and a Helltide with no luck finding the Red-Cloaked Horror.
Instead an insane huge cinders dropping from the blood maiden. Around 1K each maiden run. Crazy

The blog says they have a CHANCE TO SPAWN and CHANCE TO APPEAR. 
From November 19, 10 a.m. PST—December 3, 10 a.m. PST, the Red-Cloaked Horror has a chance to spawn in Helltide, instead of a Hellborne, while Blightfiends spread their malice within Helltide ambushes. The Red-Cloaked Horror also has a chance to appear in the Infernal Hordes and Dungeons to ambush you from the shadows. Defeat these frigid foes for increased rewards, such as Cinders, Burning Aether, and more.
What’s he drop? 10 raw hides in 10 different piles to make it look like it’s worth it?

He spawned for me within 3 minutes of entering a Helltide.

I got him twice in the same helltide this morning.
He tends to spawn as the Hellborne that spawns on you after your meter fills and depletes. He also tends to spawn in Hordes if you earn enough to spawn the Butcher or select the Butcher as an option.

Man, I don’t know what your doing, I’ve fought him like 7 times, he’s so annoying (all from hell tide).
I don’t know why, I can’t even get any rewards from it for some reason…
Did you check the chimney? Sometimes he gets stuck.
Yes but he doesn’t drop anything of value in hordes unfortunately so for me it’s helltides. It’s all about the threat meter. Keep filling it up and fyi staying with the Blood Maiden does not do that. Take those cinders and keep killing stuff while you run around opening chests
He just tried to spank my little Rogue in a dungeon full of elites I was already fighting. It was rough but he eventually fell down.
