In the closing D4 cinematic Nyrelle is in the boat with the stone and the Blooded Wolf is with her.
In the VoH cinematic she still has the stone but the Blooded Wolf (Mephisto mouthpiece) is now gone. 
Maybe it jumped into the water
She threw a woodstick to the water
Yeah she probably figured the wolf would chase the stick and then she’s free of any responsibilities!
