There is a very big problem with the wallpaper engine of Steam. If you tap out of Diablo with Alt+Tab, for some time (a few minutes, sometimes even a few seconds) and re-enter the game, then the game is permanently in FPS drop. The only thing that helps is a restart of the game.
Had the same problem and after I uninstalled Wallpaper Engine, the problem no longer occurred. In the background to close the engine or in the task manager is not enough, you have to actually uninstall it at Steam, otherwise it will continue to occur with the FPS Drops
