I have the item “Ferocious” Magic Mace dropped, with the pink square instead of the weapon visual. It says “PH” inside the square. When equipped, the item looks fine but in the inventory it is just a pink square with PH letters inside it.
When it dropped, the item wasnt visible with text, and found randomly as it didnt show as loot.
