I unlocked my Tier 9 Unique Coven Crate, opened it and did NOT get a unique. Text says “includes a guaranteed Unique”. I only got 3 Legendry and 1 blue item.

Same bug and item drops on Xbox.
This also happened to me a minute ago, 3 Legendaries and 1 Blue.
Same. I was not in Torment difficulty, if that is any help.
Patch 2.1.2 even has a note that it was fixed, but I guess they did not fix all the ways this bug can trigger.
They just released another patch. No mentioning of the bug being fixed
