Give it a day or so and we will have pictures of stuff that is not data mined. Harlequin Crest is in D4. For the most part, you will not see uniques until WT3. The Butcher spawns randomly but he is always level 25. So, getting his Cleaver is still a major achievement.
I would assume that and shaco are not on the loot table currently, they are hidden. In D3 Blizz had a rotating loot table, meaning certain items were not allowed at certain times or had “soft” unlock caps in the game. These things dont come up in patch notes but anyone who plays for years realizes that Blizz creates hidden break points for gear to drop.

My assumption now is that they only drop in T100, and there aren’t enough players doing that content yet.
Let’s say there are multiple thousands of people who have been farming WT4 for days now, not a single Shako, Grandfather and Andariel visage drop have been confirmed.
They are not in the game right now, this is not how Blizzard operate with their game.
Yes, anyone with early access has had time but they put in a patch on 04 June. Also, it was yesterday (?) that we had a streamer get to Hardcore level 100 and the day before that when a streamer hit regular level 100.
So, not many people are looking for BiS gear, etc. as they are just trying to get the first toon to 100. As such, today is the first day people will grind for gear and be able to tell us what is out there. Although, I was wrong about how long the race to 100 would take so…
