Post-campaign new character created.
I’ve done every other side quest except this one (standing at 34/35) because it won’t show up. He’s just standing there next to the Alchemist, there’s no side quest marker.
EDIT: The quest will neither show up on a fresh, post-campaign character, or a brand new VOH campaign character either. As a completionist, this is devastating LOL
