Doing Nightmare Dungeon Tier 47 at Pallid Delve (Slay the Headhunter and Collect the Salt Encrusted Key). I can’t be 100% sure if the headhunter spawned or not, but at the very least no Salt Encrusted Key has dropped, it’s not in my inventory, on the ground, and I can’t continue the nightmare dungeon. There are zero enemies left or things to interact with as well.

Currently having the same issue. Cleared the dungeon and there is no headhunter or key.

Same just happened. All enemies down. No Headhunter, so no key. NM T39
I had the same issue. - I’m not sure if the headhunter did show up but there’s definitely no key.
Same issue, triple checked cause I thought I missed the drop, nothing in here and the door is locked.
