Is Nyrelle just human form of Lilith? Why did she decide to put Mephisto into the world stone? I’m interested in seeing where this story goes. I noticed the diablo flashes during the last cutscene.
Things were being manipulated by Mephisto from the beginning of the game. It’s possible he even had something to do with Lilith being released so that Lilith led you and Nyrelle to him but that’s not certain. Another possiblity, the more obvious one, is that he acted and got our main character involved ‘because’ Lilith was summoned.
Seemingly, Lilith knew what Mephisto was trying to do in the long run (we don’t know what that is yet) and was on her way to stop him to “save Sanctuary” in her messed up way. Defeating her might have been a mistake. She was bad but what she was going to prevent is apparently much worse.
Nyrelle put Mephisto in the world stone because she saw that he was manipulating things and wanted to contain him, since he’s a Prime Evil and is much more dangerous than Lilith. She is probably correct about him being the more dangerous one. The problem is that is exactly what Mephisto wanted her to think and she did exactly what he wanted her to do; take him out of Hell away from Lilith and into Sanctuary. Reverse psychology.
Mephisto was technically in a win/win situation at the end. Either you stop Lilith from interrupting his long term plans by using the stone on her or Nyrelle helps him to escape Hell. He might not have banked on Nyrelle being there but he certainly took advantage of it since what she did was the preferred outcome for him.
Nyrelle, now, is kind of like Aiden (The Dark Wanderer) was after Diablo 1. Not quite as corrupted since she didn’t absorb the world stone like Aiden did but still at risk of being under the influence of a Prime Evil at some point. We can see that having the world stone is affecting her.
That’s my take on it anyway. I could be wrong about some things. I’m also interested to see what happens next. Going to be a year or two before we find out though. The devs said only the paid expansions are planned to continue the main story.

First: It is a soul stone, the world stone was destroyed at the end of D2 LoD.
Secondly: Nyrelle is quite ignorant of the past, for all her claims of studying ancient Horadric text she missed few key points.
The prime evils over time can corrupt whom ever holds their soul stone.
Mephisto himself corrupted over time an entire order whose sole goal was to keep him bound forever.
The Dark Exile, in D2 we learn from Izual the dark exile was initiated by the three in order to get access to soul stones. The Prime evil actively sought them.
The best way to deal with the Prime Evil is vanquish them on the spot and give Sanctuary more time without them.
If you do insist on trapping them than punt them into the void, see them corrupting anyone from there.
Further: The reason the prime evil are so power is because they feed each other. Hatred >> Terror >> Destruction >> Hatred.
Letting Lilth eat her daddy might break this cycle and unbalance the other two.
Unless Lilth eat the other two as well, but we don’t know the status of the other two so hard to pass judgement on that.
In the end, Mephisto won.
Last:
Personal theory, Nyrelle was corrupted by Mephisto back at act 1, when he helped us to rescue her from the Horadrim trap. I think Nyrelle failed some test the Horadrim of old put in place to prevent the unworthy and fools accessing their power.
