I just used the cube to upgrade The Undercrown to a mythic unique, instead it converted it to a mythic version of The Unmaker and wont even let me equip it (tbh I just want my Undercrown back!)
Did some tinkering around and you cannot equip two crafted mythics. I just removed one of mine and I can now use the weapon. Doesn’t solve your issue about the loss of the item but may allow you to use the one it crafted in the meantime.

I’d not tried with crafted mythics, thats a good shout, strange that it will let me equip other mythics, just not a 3rd crafted one! (I’ve just tested as well, thanks very much Revanessa).
