I watched one of the runs that is posted on the ladder for pits on Maxroll and the guy had two active potion icons and an incense. How is that a thing? Are there any other active or passive skills that use the same icon?

The 2nd elixir is from a quest reward. It gives 15% max life and some poison resist. For whatever reason it stacks with ither elixirs.
