At level 82 and 3 bubbles in, I am missing 3 paragon points. I’ve gotten all 20 points from paragon and the 4 from Lilith altars. I should have an additional 132 from my levels. All in total I should 156 points available to spend but only have 153 after I refunded every single one of my points several times now.

Every level from 50 will give you 4 points. There are 33 full levels in the range from 50 to 82 (you have 3 bubbles on 82 so the next point would be given at level 83). 4 x 33 =132. 132 + 20 (renown) + 4 (altars) = 156.
Having the same issue after changing spec.
The game loses track if you remove a node too quickly. I am missing a point after changing spec.
This needs to be fixed.
