I salvaged two Mythic items (Andariel’s Visage and Doombringer), and the Blacksmith crafted the Mythic Cache using the two Resplendent Sparks. However, when I attempted to open the cache, it simply disappeared without dropping any loot.
(It happened on my hc toon on torment 8 right now)

Same thing happened to me 10 minutes ago.

The same thing has happened to me on 3 separate occasions, meaning I have had 6 sparks “eaten” by the game.
