So far I’ve had the following happen, compiling as it’s likely all related - these were all seen on T4 with a Sorc.
- Multiple treasure goblins with no loot
- Living steel chest with no steel
- Arvice world boss with no drops
- Helltides gear chests with no gear
- Grigori didn’t drop a shard

Edit: seems it’s happening with Varshan eggs too.
Edit: obelisks don’t always drop elemental cores, which could be a bug - not sure, as it is a new mechanic
