Like I’m trying to farm these bosses but farming out these materials is brutal enough to make me want to quit.
Already level 100. Trying out a new minion build or rather WANT TO try it.

I’m sure you’re aware, but just in case I’ll ask the obvious stuff first. You know that Malignant Hearts can only drop in World Tier IV? And you know that Distilled Fear drops only from 30+ Ancestral NMD?
Malignant Hearts are a bit RNG when dropping, but I’ve found them almost as likely to drop as other body parts from Whisper Caches and random Grotesque Debtors in the overworld. The caches generally give me ~3 random body parts and Grotesque Debtors give me 1-2 random body parts. And thus far, two Grotesque Debtors always spawned after killing a world boss for me. In my experience, I’ve had the body parts drop a lot including Malignant Hearts. My first character’s level 85 now and I have the materials to do the Varshan fight 15 times. I’m sorry you’ve had really poor luck with the Malignant Hearts, guess it’s super bad RNG in your case.
Distilled Fear is by far the most difficult material to get out of all boss materials, I’ve been able to fight the others multiple times, but after ~35 NMD I finally have enough Distilled Fear to fight The Beast in Ice for the very first time. It’s pure RNG and I find them not dropping often enough by far, it’s really unfortunate.

I agree, drop rates are awful, and I am not bashing my head against a wall trying to get these items to drop. I am doing tier 4 content and 60+ NMD’s and still can’t get them to drop. Play something else until they fix it.
I thought only Varshan drop them malignant rings, why do you need bottled fear?
Just do world bosses and run around blood harvest for the hearts and parts

I have a hard time finding the gurgling head and trembling hands lol
I am also find the drop rates on the hearts to be very lacking. I have so many of the other items which are useless without the hearts. Would be nice if we could convert one of each to a heart. Or better yet 2 to 1 so we can at least farm Varshaun a little more consistently.

I’ve been doing Blood Harvests, Bosses, and Legion Events for 2 days and not a single heart has dropped. So fricking frustrating.
they only drop on world tier 4
You can craft the Hearts with 1 of each of the other body parts that you can just get from the Whisper cache.
