I linked by battlenet account to my psn account before all this… I have done all the things needed to get all 4 beta rewards. I did the second beta weekend got 2 lvl 25’s got to kyvashad cleared the first act, after that i did the server slam. Got to 20 and killed ashava. I have gotten the achievement for killing her in the serverslam for my horn but i am missing every reward. Ik really bummed out about this. What can i do?

Having the same issue (PS5) trying to resolve it also.
Same here.
After I connected PSN to Battlenet I could go longer log in.
Purchased PC version to rectify that, but that wiped my character and all prices including server slam rewards.
Also happened to me, PS5. Completed all beta and Server Slam achievements but now they’re gone.

Finally heard back on my ticket and was pretty much told if I don’t have it now, I’m out of luck…
I played all betas and got all achievements for the rewards but my beta rewards are missing… I did just realize that I did not have my PS5 linked to my Battle.Net till AFTER the Server Slam. Did I screw myself?

Any rewards gained through the Beta would only be applied to the Battlenet Account you had your console profile linked to at the time those rewards were earned.
If your account was not properly linked to a Battlenet account or was unlinked and then linked to another Battlenet account those rewards would remain with the original Battlenet account they were earned on. (Or not provided if not properly linked)
Support cannot see your eligibility, nor can they see any of your console data. They can only see what licenses are attached to your account. So if they did not see them and you do not have them, you will not receive them on the account you are currently using.
Please note this goes for any of our games that have cross platform progression. All progress is tied to the Battlenet Account once linked and cannot be moved, reset or duplicated to another account. If you unlink your console profile and link it to a different Battlenet Account that progress and those licenses will not move with you.
So, same issue for me on Xbox. Lost all my game progress and pre-release beta rewards because I linked my battletag after getting them? When it wasn’t a requirement (or even a suggestion) to do it before?

So if I understand correctly. If I bought ultimate package on ps and then regular on pc and linked my accounts during server slam and saw my account worked fine. Then regular release came out and I can’t play diablo 4 on my pc from the battle.net account linked to my ps account, which isn’t the account that paid for the game, but this didn’t affect me during server slam. So I changed my email addresses on the two accounts so the battle.net account that bought the game would be the same as the one for my ps account. I linked the correct account as it displays when loading the game. Linking the second account wiped all my beta, and server slam accomplishments away. It also wiped the character I beat the game on. It also wiped my diablo 2 resurrected characters as well. Is the message I should expect to get along the lines of I am screwed and they couldn’t be bothered to fix this issue? or is there an expected patch in the works? I guess I am just losing a couple of titles, a horse, and a horn. But that is my $20. Man I forgot how dealing with Blizzard is just no fun.
I didn’t switch anything, I still dont have my rewards. I pre-ordered the game, set it up through my battle.net account for my PS5, played the server slam on my PS5. Opened the game on release day and didn’t get anything…?
I participated in server slam, got the thing saying I will receive the rewards, have not done anything weird, have not received rewards.
