you missed that one.
the one on your map near that location is the one is Scosglen. You missed that one, which a little bit west of it.

It is weird that people always seem to miss the same altars.

They usually miss the same ones, or count altars in other regions.
I think the in-game map needs a bit of a change when differentiating regions. The minor color difference just doesn’t seem to cut it most of the time, and unless you mover over every zone, there is no other way to tell.
Give us dotted lines between the regions or make the color differences more noticeable.
