Ive been having this same issue on 2 different Gaming Computers. Recently built a new gaming PC only to prove what I already knew still existed, this stuttering problem is a Server RTT Sync Issue and Memory Leak.
I started playing Diablo IV on my now 2nd Gaming PC. Town hubs are generally a stutter or lag issue, especially when portaling in or entering/leaving on foot/horse. Over time the game just starts stuttering increasingly. Especially when opening the World Map or Dungeon Map. Also due to the game not having a dedicated Fullscreen Mode, Alt-Tabbing increases the memory leak exponentially, each time, as its immediately noticeable with the increased stuttering and hitching. It gets so bad that the in due time you wont be able to get updated animations on screen or ultimately CTD.
Id get an average of about 75 fps on the old rig.

Now once I built my new Gaming PC the stuttering calmed down quite a bit, but it didnt stop. My sessions are probably twice as long until the stuttering starts to become cumbersome. Now Alt-Tabbing in and out, the memory leak build up still increases and you start to notice the stuttering and hitching right away.
This rig I get 130-165+ fps with everything maxed and Reflex On.
So even though my old gaming PC is dated, it could still run Diablo IV without issues at max settings if these leaks didnt exist.

The rest of the issue exists on the Games Server(s) and Network Coding. Im on 1Gbps Fiber. Besides the DDoS attacks that have been occurring, during the gaps between where the connectivity is somewhat stable there is desyncing between the player actions and server rendering back. Others have identified certain skills also lagging or activating multiple times when its an abilities that has charges (i.e. rogue skills like Rapid Fire or Precision Shot). There is also Micro Rubberbanding and your character attacking in the wrong direction when your cursor is pointing the correct direction.
Also seems like some sort of aim-assist meant for console players is constantly active because sometimes your character will start to attack the nearest enemy which could be to its sides or directly behind instead of the one thats in front you are targeting.
Edit: Also Diablo IV runs CPU’s and GPU’s very hot. I thought my older PC was just needing new Thermal Paste and a cleaning as it was running at 85-89C. Cleaned and Repasted the CPU and it still runs Diablo IV fairly hot at about 77-85C. The Load on the old CPU is 90-100% for some reason even though I have more than enough RAM and GPU VRAM to accommodate the game.
New PC runs Diablo IV at 74-83C. Load barely reaches 12% on the CPU, yet its this hot.
So its not necessarily an issue with either PC, its an issue with resource management as a lot of the Processing required is directed at the CPU rather than the GPU. Any other high end game I have barely makes my CPU Temp go above 65-67C average. Some hit 70C. This is on both PC’s. Diablo IV is the only one that gets this hot.
