I just unlocked the Primal Helm transmog and have it applied, whenever you die the face covering goes crazy. This only happens on death and when using the campfire at legion events from what I can see. I would love to upload an image and be able to link it so you can see what I mean, but I am unable to include links in my post.

This bug is also happens during world boss fights, the cloth covering the face looks like an elephant’s trunk. It’s sad because this is the best looking helm, but this bug just makes the character look utterly stupid during boss fights.
