There is an inaccessible undiscovered area between South of Amber Sands and Northeast of Caldeum. When standing at the closest point, the location name “Forsaken Gate” pops up. Any idea how to discover it?

It is probably the entrance to the cow level. You need to find all the pieces to unlock it first.
Rhyyker talks about it from 5:45 to 11:45 in this video. All speculation, of course.
Hmm, I see. So I assume it is not counted toward the map completion then? I also posted the link to the screenshot.
Well, in your particular instance, you need to just access that area from the north. You have not fully uncovered everything.

I tried, but it led to a broken bridge and I couldn’t go across 
. I really hope it’s not because I finished the campaign before visiting that area where an event happened to the city and changed it. Do you already have it unlocked before Act 5/6?

Edit: Turns out it just says undiscovered, but actually counts.
Can you get as close as you can to the area, and take some screenshots?
Exact same problem as OP. I am collecting all the Altars and clearing all of the fog of war, just to find this one tiny area near the bridge as undiscovered.
I followed your post above, but nothing happens.
In my game, when I go right to the end of the Bridge, the name changes from “Road to Alcarnus” to “Forsaken Gate”.
When I go to the map, It shows that area as discovered, but then it starts to fade, and reappear. I think that as long as you have been there, you get credit for it.
Maybe you are missing another area?
Take a look at this clip where the map starts as faded, then re-appears.
For these screenshots, I did not move or anything. I just stood still and waited for the map to clear, and then re-fog.
