The only thing you are locked out of is the new zone, new class, Undercity, Dark Citadel, Mercenaries, and the Rune system. Quality of legendaries remains the same regardless if you have the expansion or not, you just can’t further empower your character since you don’t have access to the rune system.
The welcome back package is best used at level 1 as it will boost any character immediately to 50 (Eternal Realm only, but does not work on Hardcore). It would be pointless to use on any character that is already level 50 or above. After that it’s up to you to pick a difficulty.

Sadly you tend to out scale the enemies early on. So normal through Penitent difficulty probably won’t pose much of a problem to you, at least while leveling. Once you reach level 60 and complete Pit Tier 20, you can unlock Torment 1 and begin that journey. Every 15 levels after Pit 20 will unlock the next Torment tier of difficulty.
So for example to unlock:
Torment 2 = Pit Tier 35
Torment 3 = Pit Tier 50
Torment 4 = Pit Tier 65
You’ll eventually come across enemies that will pose a problem to you, you just sadly won’t reach them until you’re in the torment difficulties which is only accessible once you’re level 60.

Just play the game normally. You can go to Helltides, complete Whispers, Nightmare Dungeons, Pits, Infernal Hordes, Undercity, Ladder Bosses, World Bosses, and Legion Events. Pick your poison, there’s a lot of options.

Yes, all your items and characters will get downgraded if you don’t have the expansion. I had some OG characters that had their lvl go to 50 and their paragon boards gone even though I didn’t purchase the expansion. Pretty lame honestly.
Not exactly accurate information here. So for fear of the Op getting the wrong information let me clarify.
Any Eternal Characters you had going into Patch 2.0 (S6, expansion release) had their levels reduced to 50. Because 50 was the character level cap (not paragon cap big difference) before Patch 2.0 came out. The patch itself changed the base game for everyone, regardless if you owned the expansion or not.
The new level cap is 60, with a new paragon cap of 300. Again this is a base game change and affects all characters regardless if you own the expansion.
As such your items on those characters at level 50 were nerfed accordingly and lowered in their item level and stats. You can still use these to level up to 60, and along the way you will find upgrades to said gear.
Once you reach level 60 on any character you will then regain access to any amount of paragon points you earned previously. Again this only applies to Eternal characters.
The most annoying thing for me was the paragon boards being gone. I don’t remember what choices I’ve made back then but I was happy with how I left them be and how the build worked. After the reset I can’t really go back and play those characters the same way I left them.
You got all that switch when going from S5 to S6 because of a full reowrk of the XP system, so it was logical you could not retain the legacy boards from S5 to work in S6 or eternal.
The XP rework was a game wide change, not only for the expansion.
You could carry your old legacy gear, but to have legacy XP boards was a bit unrealistic…
All the D franchise has gone that way, when a major rebuild is done, so nothing new here…
