I went to Torment 1 level to kill Lilith in hardcore mode. I killed the first part of Lilith. In the 2nd part, while walking around the platforms, it dropped the side platforms. When I was going to kill Lilith, it turned the middle platform red like the side platforms and killed me. The error is caused by the game and since my character is hardcore, it does not revive and my items are gone. I kindly ask for your help regarding the issue.
