I randomly experience game crashes with the following error:
“Graphics initialization failed, please ensure that your drivers are up to date and no other 3D applications or games are running and try again”

My game crashing turned out to be an unstable undervolt/overclock on my video card, I suspect specifically for the VRAM. The settings had been stable prior to VoH, but I guess the expansion with its visual updates pushed my previously stable settings to instability. Giving my video card a tad more voltage resolved my issue.
I’ve got this too, preceeded by heavy artifacting (random glowing particles, pixelated glows, blackened textures). Can’t even start the game, it instantly crashes. Yesterday I played 6 hours with no problem at all. It’s weird and it’s utter BS.
I’ve been experiencing this problem on both of my computers, randomly for the last couple of YEARS…or so …wasn’t paying close attention as I walk away and live real life, sometimes. Some updates it seems to fix itself, but as of today (2/13/26) it is yet again, being obnoxious. I have two different NVidia cards on two completely different computers (MSI, Lenovo…don’t judge! There was a issue with my MSI case awhile back and I was in a rush so got a Lenovo at some Best Buy, somewhere as that was the best they had at the time). During the last few years I’ve traveled extensively so the internet is different, each time (so NOT that). In the last two years, I had three homes in three different cities (yes, I lived in all three at the same time, and bounced from one to another). One computer was stationary, the other moved around. All locations, both computers, and both Nvidia graphics card versions had the same problems when the problem was a problem. Sometimes, after an update the problems fix themselves, and I’m happy that someone finally figured it out. Then…like today…both of my stinkin’ computers (in the same home today) are having the same problem! It fails when too much is going on with the screen. Tried reducing the graphics to low level stupid, and it is still breaking, killing my characters (good thing I wasn’t playing hard mode). This problem has happened twice in the last couple of hours. I would probably go on and do something else, but today I’m personally sick with some virus, and I have house guests who I am trying to avoid so I don’t give them something that kills them. Can someone PLEASE get this repeated problem, fixed? I have been forced to download another game to play, because my favorite game is f*ing with me again. After all this time, I still can’t figure out what the deal is, but giving the repeated circumstances (different cards, wildly different machines, and different internet access over YEARS) I’m going to go out on a limb and say… This is a recurring Diablo 4 problem. Please fix whatever this is. oh, and I would be willing to test for you guys if you think you have the problem figured out (you should have my email). Please reach out. Thank you.
