For me it dropped when it spawned as a hellspawn, did not drop when it replaced the butcher
Thought it was a chance to drop, I could be wrong though.
Agree does not drop when you choose the butcher option
The more I learn about this event the more I just facepalm. Unbelievable ineptness.

For additions like this, when putting them into Infernal Hordes or (in the future) Undercity, making it spawn an extra chest with the item(s) that would normally drop if the mob were elsewhere would be prime. That’s what they should have done here. It should have been made to function exactly like the treasure goblins in Undercity.
