Game Crash: Graphics initialization failed

I randomly experience game crashes with the following error:
“Graphics initialization failed, please ensure that your drivers are up to date and no other 3D applications or games are running and try again”

My NVIDIA driver is up to date (this error also occurs when using 566.36), and I have no other games running.

I can sometimes play for over an hour at a time without this crash, while sometimes it occurs after less than 10 minutes in-game.

Update: since installing Window 11 KB KB505849, I haven’t had any crashes in over 10 hours of play time.

Coincidentally, Windows 11 24H2 introduced that bug the same week VoH launched. VoH launched with bugs, the same week Windows 11 updated with a graphics bug, with NVIDIA drives becoming very buggy 4 months later.

Hi, I know this thread is over 20 days old, but I have a question about this crash.

Have you tried disabling your Intel UHD Graphics 770 adapter in Device Manager and seeing if that stops your crashes?

Normally this can also be achieved by telling Windows which GPU to prioritize in the graphics settings as well, but it may not be working correctly.

Although a lot of people are having this “Graphics Initialization Failed” error, it might be a Windows issue that occurs under certain frame circumstances with different games.

I appreciate the suggestion! Unfortunately I already have the iGPU disabled in BIOS.

Like many others experiencing bugs, I never had D4 crash until VoH, whereupon I got regular Fenris crashes to desktop. Those crashes seem mostly resolved, but I now get this “Graphics Initialization Failed” error every once in a while, which seem to have started this year. When I searched, I found lots of forum and Reddit posts about that particular error.

similar issues here. Made/Making more posts until blizzard does something about it. (2 years waiting)
literally freshly installed diablo, and encounter this issue in after a few hours. really lame.

Most updated drivers dont help, older 566.36 doesnt help

no devs even acknowledge this issue after almost 2 years of it being a thing for many people.

Same Issues here! Just bought the full game with the two expansions, expecting to receive a stable version, and this graphics initialization issue is making the game unplayable.

My configuration here: Ryzen 7 5800X3d, 7900XTX 32gb, 32GB Ram DDR4, 2TB SSD.

Only D4 has this issue.

Any help would be appretiated.

Same issue for me, bought the game and it constantly keeps crashing with that error. Tried reducing texture size, graphics settings and resolution. Nothing consistently worked. For a day I thought raytracing was causing this, because I had it enabled after buying the game. Graphics drivers are up to date, but for good measure I went back to a random older version some months ago.

Playing on 7800x3d with 64 GB of RAM and RTX5090 on a 4k display locked to 100 Hz (some older thread was saying to keep it lower (I could do up to 240 Hz)). I am still on Windows 10 but people seem to report this issue pretty independently over hard- and software and also time (since years lol).

Pretty sad because I really would like to play this game extensively.

Just started having this issue after Lord of Hatred update. Was entirely fine before, now i freeze and periodically crash with the game telling me I have the same issue. Has this been addressed for anyone?
