I’ve spent the past month trying to optimize my game settings. I got a new video card. I tried different vsync settings. Ultimately, D4 is way more stable at lower FPS. I set my monitor refresh to 144hz and set vsync to adaptive half refresh. No regrets. Don’t get me wrong. I have a nice computer. The game is just very demanding on hardware. I’m posting this as a suggestion for anyone else who’s in the same boat. Just lower your FPS. 120+ FPS is a foolish goal unless you have a 6k dollar PC.
Okay but why did you have to name your topic like this

The game has a VRAM leak issue that’s known by the devs. It sucks, and makes your GPU work much harder than it should have to for this game.
I’ve got a 6950XT on a custom water loop, the thing never gets above 40c and around 38% use, but my 16gb of VRAM is always at 100% use, and seems to be the sole limiting factor currently. It’s a beast in every other game, and frankly overkill everywhere except in D4.
Hopefully it gets fixed soon, because I’d love to crank up my FPS a bit.

Lol, sorry. It wasn’t a well-thought-out title.
