After slaying all enemies in an area, an elite enemy holding a key spawns. However, the elite enemy spawned behind the locked door that I needed the key for. Leaving the dungeon and re-entering did not fix it. I was able to kill the elite behind the door with some abilities, but that just dropped the key behind the locked door, just making the situation worse, so I abandoned the dungeon. It is also worth noting the elite enemy had the [teleporter] perk, but did not seem to change anything; it was unable to teleport through the door. This occurred in the “Cultist Refuge” dungeon.
