Pointing to other thread because 2.0.3 did not fix it. There are a handful of people who seem to have received one but the majority as you can see clearly per this thread below, are not. Self included.
Additionally, some kind of official ‘we are onto it’ would be nice.

Yes I did another fresh character kill who had not, and same thing, again.
The longer they leave it, the more likely they will need to reset everyone’s kills. They’ve already managed to annoy a lot of people but that achieves nothing.
Same here. Killed her for the first time EVER about 20 minutes ago. Two pieces of garbage loot and no spark. Really makes the 40 attempts worth it.
I also finally defeated Uber Lilith for the first time last night (10/23 @ ~2:00am PT) and no spark. I also just learned that a spark is given for first time kills on the tormented versions of Lord Zir, Grigoire, Varshan, Beast in the Ice, Andariel, and Duriel. I cleared the first 4 in T1/T2 multiple times but I only have 1 spark in my inventory. Please fix soon

because I killed her and I did not receive it.

Wow, that’s new. I got not one spark from any of them. If that is correct then they probably need to address it because it is not good.
Yes it is expected. It is a programming/development error and they know it, and no, they have not fixed it unless the current patch update has. I guess we will find out soon enough.
Same issue for me today. Killed on T1 and then T2 thinking T1 wasn’t high enough and still no RSpark.
Killed Lilith on T1 and T4 today, no spark.
You only get 1 spark for your FIRST tormented boss, not for every one. So, 1 Spark for the first Tormented boss killed of the Season. I did receive mine. BUT, did NOT receive it for killing Lillith
You are luckier than most on this thread and the linked one on the opening post.
We all received none for any boss or Lillith.

PLEASE have someone give an update as to an ETA for a fix. It is a little outstanding and no, the prior patches did not fix it and 90% of people dipped out on the spark so consider recompense or a reset on the sparks for Lillithplease. Nobody wants to fight the toughest boss in the game for zero reward other than ticking a completion box.

Killed Lilith for the 1st time less than 1h ago and didn’t get the spark – T3, game version 2.0.3.59036.
There is almost no point killing her anymore. At least with tormented bosses you get something guaranteed. The renown you get for killing her is negligible and can be gained from doing other things faster.
I’ve gotten the spark from the season journey and for killing a tormented boss, but still missing the spark from Lilith, killed her on both T1 and T4.
Well you are doing way better than a substantial number of people as can be gauged by this thread and the one I linked to.
We are now on v204 and still no mention of spark fixes in patch notes 
Unfortunately, you seem correct. BLIZZARD knows there is a major inconsistency with people not receiving what their own notes say.
No point getting upset as it solves nothing, but it is sad that they do not even acknowledge what so many people already know.
Either they are besieged or they don’t give a damn. They don’t even want to reply to all our complaints post the so called patch. I got my Lilith spark in S5 but not S6…no fix despite raising a ticket and complaining. We should not be parting with good money on game developers who cannot live up to what they say
