I’ve completed The Infernal Hordes, Defeat the Fell Council (completed in that I have defeated the Fell Council). However, the quest is stuck, as am I. Stuck in the final room of the Well of Hatred, everything dead, nothing spawning, no rewards, and unable to port out.

Yep, me too and it is certainly a bug requiring resolution.
Just beat 10 waves got 500 tokens killed the council and now stuck in the chamber without any rewards!!
