As per this short released yesterday, three conclusions can be drawn:
- First completion of all 3 wings on any Torment difficulty gives you the normal helm, gloves and boots.
- Completion of all 3 wings on Torment 4 will reward you with the greater helm.
- The rest of the normal/greater pieces are RNG loot.
I received the normal helm and gloves only (no boots or greater helm) from my first t4 completion this week. Prior to that I had never touched the citadel on any difficulty. I suspect if you complete for the first time on t4 it will not give you the greater helm because it has to give you the normal helm and greater helm but obviously has not done so for many people. This is a bug and obviously not intended as many are reporting the normal boots and greater helm not dropping because they should have been automatically granted but were not for some reason. Please make sure the normal boots and greater helm are automatically rewarded to all players who completed the Dark Citadel wings on Torment 4 after the 29th October patch.

Not from my experience. I have all the normal pieces except the boots.
It took way too many T4 runs to get my first greater piece and it was gloves.
The drop rate on these is far to low. I have wasted far too much time trying to get these.
Doing content I never wanted to do in the first place.

Maybe there is something with the drop % but I got my Greater Helm and chest my first run and it was in T4. I also got the normal Chest in the same run. My second run, I got nothing. My 3rd last night, I got normal Gloves. I never got any Boots and no mount with his armor. Ill try again tonight. Also, I didnt get the cache which seem bug…
I received normal helm and gloves (no boots) on my first T3 completion, each from a different boss. Upon first T4 completion (and ever since), no greater helm awarded. So multiple T3 and T4 completions - all 3 wings - and still no normal boots or greater helm. Please fix this!!!
Today I have an achievement for 100,000 coins of the dark citadel. So far, there are 4 items of great hazor… And it is not clear whether it makes sense to continue further… The first three things fell in the first 2-3 days, then 60,000 coins and 0 drops. Today, the pants fell off, then a few runs and 0 again. What are they stirring up, why make people nervous, I’m already sick of the citadel.
took me over 100 middle wing runs to complete the greater armour and mount armour it’s pure rng so either get rdy to grind the middle wing or forget about the cosmetics
Yeah, I’m at around 75K Citadel Coins, and I have the full Greater Khazra armour set.
But I’m missing the boots from the normal set. It’s really bugging me.
I’ve also gotten some polearms and two handed weapons, and the mount banner and mount armour. Sigh.
I ran Citadel 15 again today. Regular Khazra pants dropped, Great Khazra pants dropped yesterday. It seems like they haven’t fixed the bug. I’m still waiting for Great Khazra gloves, but they’re not here… Regular ones dropped on day 1, and now T4 gloves are closed to me, I think. Waiting for the patch again… And the most interesting thing is, I’ve been going to T4 from the very beginning, but more regular Khazra drops, not counting day 1. Brilliant, considering that regular Khazra drops on T1 too… already 107,000 coins…update 118000.
EDIT: I have received the Greater helm, normal boots and completed my collection after the patch. Many have reported they relogged and it was unlocked already, some have reported that they had to do 1 round of all 3 wings on T4 (which is what i did) and they drop when the “conquered dark citadel” message appears on your sceeen after finishing all 3 wings and claiming the weekly cache. I didnt have to open my cache the cosmetics were on the ground with the boss loot, both the greater helm and boots (guaranteed pieces).
And yet, like some on this thread, some have unfortunately informed me that they did the same as i did and did not receive the cosmetics they should have. One guy messaged me on discord saying he got the greater helm but not the normal boots, others saying they are missing the greater helm etc…
There needs to be a fix for this pronto. People who do not frequent the forums, reddit or discord are wasting their time grinding for drops that will never come. Be transparent with your playerbase Blizzard. Fix. It.

I’m sure I have a mistake in getting the Great Khazor Gloves. At the very beginning, in 1-2 days of farming, I got a Great Helmet, Great Chest and Great Boots, Simple Khazor Gloves, well, and the mount armor and their accessories. Then for 60,000 (!) coins, I didn’t get anything AT ALL, 0 drops. Waited for the patch, started farming the citadel. Got Great Pants, Normal Pants, Normal Boots (which are a guarantee). This is for several runs. I think, great, the Great Gloves will drop soon. Well, that’s it, for God knows how many runs before Zagral (20,000 coins) I still didn’t get ANYTHING! Blizzard, please fix a simple thing as a drop. I have already collected 117,000 coins on my account and still don’t have a full set. And I don’t see the point in farming further, since the loot is still broken. You fixed it a bit, I got the pants, but alas, I won’t get the gloves because of your loot bug. I have the same 4/5 people running with me and NO ONE saw A SINGLE cosmetic for the whole team in 4-5 hours of farming!At least one blue one answer, is there any point in cleaning this citadel 500-1000+ times, is it intended? or am I wasting my time. I can already go through it with my eyes closed, watching the series…

As many as 100, wow. I already have ~125,000 coins. Just YESTERDAY I made 40-50 times the middle wing and 50 more times the 1st boss of the middle wing (0 drops of cosmetics, ka4 and my partners from 4/5 of the great hazor). Before that, I also made 60,000 coins (0 drops of cosmetics), you can calculate for yourself how much it is in the wings, if you are lucky that the drop for your account is not broken, this does not mean that it is not broken for all accounts…

I have done countless soul wings end boss but so far only gotten greater helm and chest 
I’m unable to get the regular “Gloves of the Khazra” which is suppose to be a guaranteed reward for the first time completion of one of the wings.
I’ve cleared Dark Citadel multiple times already on Torment 1 through Torment 3, and I have only received the regular Helm and Boots.
Anyone knows if I can still get the regular gloves if I keep farming or am I wasting my time?
You could try a new character perhaps. Might be able to trigger it that way.
I have already tried that. I leveled a brand new character and the regular gloves still didn’t drop, unfortunately. I think the first completion thing is tracked account wide or something 
At this point I don’t care about this cosmetic anymore.
I the mount/mount armor. Was hoping to get the everything, but Blizzard is asking too much. I am not going to run this content to no end trying to get the rest. None of my friends are playing anymore, forcing me to use party finder. 99% of players still have no clue how to do the content and its just a huge pain. Hopefully we can still try to get the rest next season and that they increase the drop chance significantly.
