For whoever completed torment 4 dark citadel for the FIRST TIME on season 11 (when the dark citadel was bugged), you cannot get “Greater helm of the Khazra”.
The greater helm of the Khazra is a reward for completing all 3 wings of the dark citadel for the FIRST TIME on Torment 4.
However, last season some of the bosses did not drop loot at all and the helm was never awarded.
The problem is that the game already has it checkmarked that someone has completed the Dark citadel on torment 4 already and thus will not drop it.
i have multiple accounts. on 2 of those i have completed 100% of all cosmetics including weapons and banners, mount armor and throphy.
On a fresh account, i currently have all of the greater khazra set minus the helmet. This account has completed the dark citadel on torment 4 for the first time last season, not knowing that the citadel was bugged at the time.
